OBTAIN A COPY

• Medical records can be obtained by filling out a Release of Information form (Download Form). Please allow up to two weeks to copy charts and complete any necessary paperwork.

TRANSFERRING TO OUR PRACTICE

• If you are transferring your child’s care to our practice, please provide copies of all previous records. We require these records, especially the immunization certificate, prior to scheduling a well-child physical exam.
